Replace or retrainmanagement teams that have been reported by previous employees so they don't do the same to the new next employees. Get program managers that can monitor and communicate each new line and do not release the product until the specs are approved.
Lack of leadership, lack of communication lack of following processes lack of following processes, old system with many side systems to get the job done. New replacement will not work well for manufacturing environment. It's more geared towards financial aspects of a company.. but now stuck with it
