Mr. Matthew J. D'Onofrio, also known as Matt, MBA, has been the Chief Business Officer and Executive Vice President at Evoke Pharma, Inc., since 2010 and also its Treasurer and Secretary since March 2007. Mr. D'Onofrio is one of co-founders of Evoke Pharma, Inc. He served as Principal Financial Officer of Evoke Pharma, Inc. Mr. D'Onofrio served as an Executive Vice President of Corporate Development at Evoke Pharma, Inc. Mr. D'Onofrio has over 16 years of experience in both large and small pharmaceutical firms. Prior to founding Evoke, he served as Vice President of Business Development at Victory Pharma, a growing specialty pharma company based in San Diego. He led efforts to acquire marketed brands for the growing sales force. Earlier, he was previously Director and Head of West Coast Business Development at Vertex Pharmaceuticals, a biotechnology company, directing partnership efforts associated with the La Jolla research facility as well as other corporate assets. Mr. D'Onofrio also held various commercial roles of increasing responsibility over a decade at Eli Lilly & Company, including significant experience in worldwide corporate business development. During his licensing career, he has developed and executed license and investment relationships across a wide collection of disease states and technologies with potential value approaching US$1 billion. Mr. D'Onofrio earned a B.S. in Chemistry from San Diego State University and an MBA (Finance) from the Marshall School of Business, University of Southern California.

Mr. Cam L. Garner is the Founder of Zavante Therapeutics, Inc. and serves as its Chairman. Mr. Garner serves as an Advisor of Windamere Venture Partners, LLC. He co-founded Elevation Pharmaceuticals, Inc. and has been its Chairman since December 2007. Mr. Garner co-founded Cadence Pharmaceuticals Inc. in May 2004 and has been its Chairman and Independent Director since July 2004 and May 2004, respectively. Mr. Garner serves as the Chairman of Verus Pharmaceuticals, Inc. and OrPro Therapeutics, Inc. He co-founded Meritage Pharma, Inc. and has been its Chairman since February 2008. He co-founded Evoke Pharma, Inc., and has been its Chairman since June 2007. He has been the Chairman of Zogenix, Inc. since August 2006. Mr. Garner co-founded Sunovion Respiratory Development Inc. in 2007 and served as its Chairman. He co-founded Pernix Sleep, Inc. (formerly, Somaxon Pharmaceuticals Inc.) and served as its Chairman and its Independent Director from August 2003 to June 2008. He co-founded Xcel Pharmaceuticals Inc. and served as its Chairman from January 24, 2001 to March 2005. He served as the Chairman of Actavis Mid-Atlantic, LLC (formerly, Biovail Pharmaceuticals, Inc.). He served as Chairman of Favrille Inc. from May 2001 to October 2006 and its Director since December 2000. He serves as a Director of CardioDynamics International Corporation and Ngen, Inc. She completed her Dental residency in geriatrics and special patient care at Long Island Jewish-Hillside Medical Center and Internal medicine residency at Cleveland Clinic Foundation. Dr. Carlson is board-certified in internal medicine and has Regulatory Affairs Certification from the Regulatory Affairs Professionals Society.
