Mr. Matthew J. D'Onofrio, also known as Matt, MBA, has been the Chief Business Officer and Executive Vice President at Evoke Pharma, Inc., since 2010 and also its Treasurer and Secretary since March 2007. Mr. D'Onofrio is one of co-founders of Evoke Pharma, Inc. He served as Principal Financial Officer of Evoke Pharma, Inc. Mr. D'Onofrio served as an Executive Vice President of Corporate Development at Evoke Pharma, Inc. Mr. D'Onofrio has over 16 years of experience in both large and small pharmaceutical firms. Prior to founding Evoke, he served as Vice President of Business Development at Victory Pharma, a growing specialty pharma company based in San Diego. He led efforts to acquire marketed brands for the growing sales force. Earlier, he was previously Director and Head of West Coast Business Development at Vertex Pharmaceuticals, a biotechnology company, directing partnership efforts associated with the La Jolla research facility as well as other corporate assets. Mr. D'Onofrio also held various commercial roles of increasing responsibility over a decade at Eli Lilly & Company, including significant experience in worldwide corporate business development. During his licensing career, he has developed and executed license and investment relationships across a wide collection of disease states and technologies with potential value approaching US$1 billion. Mr. D'Onofrio earned a B.S. in Chemistry from San Diego State University and an MBA (Finance) from the Marshall School of Business, University of Southern California.

Dr. Wayne Alves, Ph.D. serves as Senior Director of Clinical Operations at Evoke Pharma, Inc. Dr. Alves is a seasoned executive with a wealth of experience in leading cross-functional strategic and operational teams. Before joining Evoke, he served as the President and Principal Consultant of Sage Clinical Development, LLC, a pharmaceutical development consulting firm. Previously, he held senior project leadership and research positions at Cadence Pharmaceuticals, Inc., Valeant Pharmaceuticals, Xcel Pharmaceuticals, Inc. (acquired by Valeant), DevCo Pharmaceuticals and INC Research Inc. He has held an Associate and Assistant Professorships at The University of Virginia, University of Pennsylvania and University of North Carolina at Chapel Hill. Dr. Alves received his Bachelor's and Master's Degrees from California State University, Hayward, a Ph.D. from the University of Massachusetts, Amherst, completed a Pre-Doctoral University Fellowship at The Johns Hopkins University, and was an NIMH Postdoctoral Fellow at the University of California, Los Angeles.

Mr. Cam L. Garner is the Founder of Zavante Therapeutics, Inc. and serves as its Chairman. Mr. Garner serves as an Advisor of Windamere Venture Partners, LLC. He is a Co-Founder of Zogenix, Inc. and OrPro Therapeutics, Inc. He has significant experience as a board member of multiple publicly-traded and privately-held companies and expertise in developing, financing and providing strong executive leadership to numerous biopharmaceutical companies. He served as the Chairman and Chief Executive Officer of Dura Pharmaceuticals, Inc. from 1995 to November 2000, Chief Executive Officer from 1989 to 1995 and President from 1990 to 1998. He joined Dura Pharmaceuticals, Inc. in 1989 as an Executive Vice President and President of a former subsidiary. Prior to joining Dura, Mr. Garner served as the President of Syntro Corp., from 1987 to 1989. From 1983 to 1986, he served in several positions with Hybritech Inc., including Senior Vice President of Sales and Marketing. He also served as Favrille Inc.'s Acting Chief Executive Officer from August 2001 to February 2002. In 1998, he co-founded DJ Pharma Inc. and served as its Chairman until it was acquired by Biovail Corp. in 2000. He co-founded Verus Pharmaceuticals in November 2002 and served as its Chief Executive Officer since July 2004. He co-founded Elevation Pharmaceuticals, Inc. and has been its Chairman since December 2007. Mr. Garner co-founded Cadence Pharmaceuticals Inc. in May 2004 and has been its Chairman and Independent Director since July 2004 and May 2004, respectively. Mr. Garner serves as the Chairman of Verus Pharmaceuticals, Inc. and OrPro Therapeutics, Inc. He co-founded Meritage Pharma, Inc. and has been its Chairman since February 2008. He co-founded Evoke Pharma, Inc., and has been its Chairman since June 2007. He has been the Chairman of Zogenix, Inc. since August 2006. Mr. Garner co-founded Sunovion Respiratory Development Inc. in 2007 and served as its Chairman. He co-founded Pernix Sleep, Inc. (formerly, Somaxon Pharmaceuticals Inc.) and served as its Chairman and its Independent Director from August 2003 to June 2008. He co-founded Xcel Pharmaceuticals Inc. and served as its Chairman from January 24, 2001 to March 2005. He served as the Chairman of Actavis Mid-Atlantic, LLC (formerly, Biovail Pharmaceuticals, Inc.). He served as Chairman of Favrille Inc. from May 2001 to October 2006 and its Director since December 2000. He serves as a Director of CardioDynamics International Corporation and Ngen, Inc. Mr. Garner has been Director of Pharmion LLC since May 2001. He serves as Director of Oncternal Therapeutics, Inc., Adigica Health, Inc., Neurelis, Inc. and Aegis Therapeutics, LLC (formerly, Aegis Therapeutics, Inc.). He served as a Director at Tokalas, Inc. He served as Director of SkinMedica, Inc. since January 2002, MMRGlobal, Inc. from December 2000 to January 2009, CancerVax Corp. from February 2001 to July 20, 2005, Dura Pharmaceuticals, Inc. from 1989 to 1995, Spiros Development Corp. since 1997 and Safeskin Corporation since June 1996. Mr. Garner received a B.A. in Biology from Virginia Wesleyan College and an MBA from Baldwin-Wallace College.

Dr. Marilyn R. Carlson, D.M.D., M.D., RAC. serves as Vice President of Medical, Regulatory and Scientific Affairs at Agility Clinical, Inc. Dr. Carlson serves as Senior Advisor of Medical & Regulatory Affairs at Proprius Pharmaceuticals Inc. Dr. Carlson has been the Chief Medical Officer of Evoke Pharma, Inc., since December 2, 2013 and served as its Advisor. She served as Vice President of Medical and Regulatory Affairs at SynteractHCR (formerly Synteract, Inc.) and was responsible for medical, regulatory and safety services. She served as Medical Advisor for Ridge Diagnostics Inc. She has more than 20 years of industry experience in the development and post-marketing support of drugs, devices, diagnostics and biologics. She founded entreMeDica Inc. in 2004. She served as Chief Medical Officer, Vice President and Consultant for companies of various sizes. She served as Vice President of Medical & Regulatory Affairs and Chief Medical Officer of Prometheus Laboratories Inc. She served as Vice President of Clinical & Medical Affairs and Chief Medical Officer for ISTA Pharmaceuticals (Formerly Advanced Corneal Systems). Prior to ISTA, Dr. Carlson served as Vice President of Clinical & Medical Affairs at XOMA, a biotechnology company. Prior to XOMA, she served at Procter & Gamble (P&G) in Cincinnati Ohio and worked in Over-the-Counter Health Care and pharmaceuticals. While with P&G, she had responsibility for Medical Affairs in the U.S. and Canada and served as Worldwide Technical Brand Manager for an osteoporosis therapy. She has a BA in anthropology/biology from Hunter College of the City University of New York , a D.M.D. from the Harvard School of Dental Medicine and an M.D. from Case Western Reserve University School of Medicine. She completed her Dental residency in geriatrics and special patient care at Long Island Jewish-Hillside Medical Center and Internal medicine residency at Cleveland Clinic Foundation. Dr. Carlson is board-certified in internal medicine and has Regulatory Affairs Certification from the Regulatory Affairs Professionals Society.
