Mr. John L. Erb has been the Chief Executive Officer and President of CHF Solutions, Inc. (also known as Sunshine Heart Inc.) since March 2, 2016. Mr. Erb is Co-founder of NuAx, Inc. and serves as its Chairman and Chief Executive Officer. He is Senior Advisor at Wombat Capital and Medical Device Cardio Vascular. He served as Interim Chief Executive Officer and Interim President of Sunshine Heart Inc. since November 30, 2015 until March 2, 2016. Mr. Erb served as Chief Executive Officer of Harbinger Medical Inc. He served as President and Chief Executive Officer at Sulzer IntraTherapeutics Inc. He has over 35 years of experience in the medical device industry with both device companies such as American Hospital Supply Corp., Cutter Laboratories, Inc., Johnson & Johnson and Pfizer; and venture capital financed start-up companies. Mr. Erb served as Vice President of Worldwide Operations of Schneider Worldwide, a division of Pfizer, Inc. He served as General Manager of Schneider U.S. Stent Division. Mr. Erb has also built two venture capital financed medical device companies; IntraTherapeutics, Inc., a peripheral vascular stent company acquired by Sulzer Medical in 2001 and now the primary stent product-line of Covidien's EV3 division; and CHF Solutions, Inc., acquired by Gambro Renal Products in January 2010. Mr. Erb served as Chief Executive Officer and President at CHF Solutions, Inc. He has been Chairman of CHF Solutions, Inc. since September 12, 2012. He serves as Non-Executive Chairman at Osprey Medical Inc. He served as Chairman of Vascular Solutions, Inc. since May 2011 until February 17, 2017 and its Director since October 2002 until February 17, 2017. He served as Chairman of CHF Solutions, Inc. since January 2007. He served as Chairman of Munson Healthcare, Inc. and VivoMetrics, Inc. Ms. Claudia Napal Drayton has been the Chief Financial Officer and Secretary of CHF Solutions, Inc. since January 2015. Ms. Drayton served as Chief Financial Officer and Senior Finance Director for Medtronic Inc.'s Integrated Health Solutions Business. Prior to joining Sunshine Heart, she spent the past fifteen years at Medtronic, Inc, a $17B global leader in the medical device industry. During her tenure, she held multiple senior managerial finance positions culminating with an assignment in Europe serving as Chief Financial Officer of the Peripheral Vascular Business from 2010 to 2012 and most recently, as Chief Financial Officer and Senior Finance Director of the Integrated Health Solutions Business. In these capacities her responsibilities and experiences included profitability management, strategic planning, mergers and acquisitions, planning and forecasting and implementation of financial best practices. Before Medtronic, Ms. Drayton was an Audit and Business Advisory Manager at Arthur Andersen for seven years. She holds an MBA from the University of Minnesota's Carlson School of Management, a BS in Accounting from the University of Mary Hardin-Baylor and is a Certified Public Accountant (inactive).

Ms. Megan Brandt has been Senior Vice President of Operations at CHF Solutions, Inc. since may 26, 2017 and served as its Vice President of Regulatory Affairs & Quality Assurance since August 16, 2016 until May 26, 2017. Prior to joining Sunshine Heart, Ms. Brandt served as Vice President of Quality and Regulatory Affairs at Cardiovascular Systems, Inc. At CSI, she successfully facilitated domestic and international clinical study submissions and secured multiple regulatory approvals including CE mark, 510(k) clearances and original PMA approval for their peripheral and coronary orbital atherectomy systems. She serves as Co-Chair for Medical Alley's Regulatory Advisory Board and is actively involved in FDA working groups including Harmonization by Doing and MDEpiNet's RAPID programs. In 2014, she was nominated by Minnesota Business Magazine as an Emerging Healthcare Leader. Ms. Brandt earned her B.S. in Biochemistry and Microbiology from Minnesota State University-Mankato.

Mr. Gordon S. Weber, J.D., has been Vice President and General Counsel at CHF Solutions, Inc. (also known as Sunshine Heart) since May 15, 2017. Mr. Weber served as Vice President, General Counsel and Secretary of Vascular Solutions, Inc. He served as General Counsel at Vascular Solutions Inc since February 2013. He served as Associate General Counsel ??? Commercial for Smiths Medical from October 2010 to February 2013. Before joining Smiths Medical, Mr. Weber practiced law for 13 years and served as a Partner in the Corporate group at Faegre Baker Daniels LLP (also known as Faegre & Benson LLP). He began his career with the accounting firm known as KPMG. Mr. Weber practice is focused on public company reporting, corporate governance and compliance, general corporate counseling, mergers and acquisitions, securities offerings and development, licensing and supply arrangements. Mr. Weber generally works with companies in the software, technology, financial services and medical device markets. He assists publicly held companies in preparing periodic reports and proxy statements, in responding to Securities and Exchange Commission inquiries, in compliance with Sarbanes-Oxley and other regulatory requirements and with general corporate governance matters. Public companies which Mr. Weber has represented include Stellent, Inc., HMN Financial, Inc., Hutchinson Technology Incorporated, CIMA LABS INC., BUCA, Inc., Angeion Corporation and Minntech Corporation. He has counseled clients on public/public mergers, acquisitions of public company subsidiaries, acquisitions of significant assets of public companies and private company acquisitions. Mr. Weber also has played a lead role in initial and follow-on public offerings, Rule 144A transactions and private placements. He has represented companies such as Guidant Corporation, Hutchinson Technology Incorporated, EZ Gard Industries, Inc., Zimmer Spine (formerly Centerpulse Spine-Tech), Minntech Corporation and GalaGen Inc. in the negotiation and consummation of complex development, licensing and supply agreements. Some of Mr. Weber's representations include representation of SurModics, Inc. in its $11 million acquisition of BioFX Laboratories, Inc.; representation of Stellent, Inc. in its $60 million acquisition of Optika Inc.; representation of RBC Capital Markets (formerly Dain Rauscher Wessels) in the $34 million follow-on offering of shares of common stock of Stellent, Inc.; Representation of International Multifoods Corporation in the off-shore placement of $200 million of its 6.602% Notes; and many more. Prior to joining Faegre & Benson, he had a 13 year career in finance and accounting. Mr. Weber spent seven of those years at Osmonics, Inc. His responsibilities at Osmonics included financial reporting, financial analysis of acquisition candidates and serving as Corporate Controller. During his tenure at Osmonics, its sales and net income grew from $46.7 million and $3.9 million, respectively, to $164.9 million and $9.8 million, respectively. Mr. Weber began his career with KPMG LLP. He is admitted to the Bar of Minnesota. He is also involved in eco education. He received his J.D., summa cum laude, in 1997 from the William Mitchell College of Law and a B.A., magna cum laude, Phi Beta Kappa, in Accounting and Economics in 1985 from the Luther College.
