Mr. Thomas William Burns has been the Chief Executive Officer and President at Glaukos Corporation since march 2002. Prior to Glaukos, Mr. Burns led Eyetech (Acquired by OSI Pharmaceuticals Inc.) as President and Chief Operating Officer. Mr. Burns was a co-founder of Chiron Vision and as Senior Vice President and General Manager, helped build that business from a start-up to a successful $310 million acquisition by Bausch & Lomb in 1998. He has had the privilege of leading world-class teams to create new commercial global markets by introducing entirely new market-classes in ophthalmology: the first foldable intraocular lenses, the first LASIK keratome, the first intraocular ophthalmic drug delivery implants (Vitrasert ), the first preservative-free artificial tears (Celluvisc and Refresh ), the first, foundational, customized LASIK technologies and the first MIGS (micro-invasive glaucoma surgery) implants to treat open-angle glaucoma. As a leading serial entrepreneur, Mr. Burns has raised a quarter billion dollars to date in private placements to build successful businesses. Mr. Burns has provided the leadership for Glaukos since its founding in 2001. He is a leader in the ophthalmic industry having built substantial value, delivered rapid growth and created entirely new commercial product classes and global markets in senior leadership roles at three major, successful start-up companies. He serves as a Director of Glaukos Corporation. He served as a Director of Eyetech. Mr. Burns received his bachelor s degree in history from Yale University.

Mr. Joseph E. Gilliam, also known as Joe, has been Chief Financial Officer and Senior Vice President of Corporate Development at Glaukos Corporation since May 2017. Mr. Gilliam was Managing Director in the Healthcare Investment Banking Group and focused on the life sciences industry, including the medical technology, diagnostics and biotechnology sectors. Since 2000, Mr. Gilliam has held various positions of increasing responsibility at J.P. Morgan, Chase Manhattan and The Beacon Group, prior to these firms being combined in 2001. Mr. Gilliam began his career in the audit services division of PricewaterhouseCoopers in 1998. He holds a bachelor's degree in accounting from the Kelly School of Business at Indiana University, Bloomington.

Mr. Chris M. Calcaterra has been the Chief Operating Officer at Glaukos Corporation since February 1, 2017. Mr. Calcaterra served as the Chief Commercial Officer of Glaukos Corporation since April 2008. He joined Glaukos in 2008, leading the operations management, strategic development and commercialization planning for the launch of the iStent Trabecular Micro-Bypass. Mr. Calcaterra has over 21 years of increasingly responsible leadership experience at Advanced Medical Optics (AMO), as Senior Vice President of the Cataract/Refractive Business. He has a proven track record of quickly growing franchise-sized businesses and successfully managing teams across multiple disciplines. His stellar professional reputation has fostered strong relationships within the ophthalmic community. Mr. Calcaterra holds a bachelor's degree from Miami University and a master's in business administration from Xavier University.

Mr. Richard L. Harrison, also known as Rich, has been an Advisor of Glaukos Corporation since May 2017. Mr. Harrison served as the Chief Financial Officer of Glaukos Corporation since January 2008 until May 2017 and served as its Treasurer and Secretary since July 2014 until May 2017. Mr. Harrison served as the Chief Financial Officer and Executive Vice President of Biolase Technology Inc. from December 12, 2005 to January 2, 2008 and also served as its Secretary from December 12, 2005 to January 2, 2008 and Principal Accounting Officer until January 2, 2008. Mr. Harrison served as the Chief Financial Officer and Secretary of Interpore Spine Ltd. (formerly Interpore Cross International and Interpore International Inc.) from November 1994 to 2004 and as its Senior Vice President of Finance since May 1998 and Vice President of Finance from November 1994 to May 1998 and also as Principal Accounting Officer. Prior to joining Interpore Spine Ltd., Mr. Harrison worked for Kirschner Medical Corporation, a manufacturer of orthopedic devices, in a variety of financial positions starting in 1987. Mr. Harrison also served as Manager of Financial Reporting; Division Controller of Kirschner Medical Corporation and as its Corporate Controller from February 1992 to October 1994. Mr. Harrison is a Certified Public Accountant and earned a B.A. in Business Administration from Towson University and an MBA from Loyola College in Maryland.

Dr. Mory Gharib, Ph.D. is Co-Founder of Glaukos Corporation. Dr. Prof. Gharib co-founded Bioengineering option at Caltech in 2000. He serves as Member of Scientific Advisory Board at 3F Therapeutics, Inc. He serves as Member of Advisory Board of Z Advanced Computing, Inc. He is the Hans W. Liepmann Professor of Aeronautics and Professor of Bioinspired Engineering specializing in Hydro and aerodynamics, biological flows, bio-inspired medical devices, and advanced flow visualization techniques. He holds more than 25 U.S. Patents in areas of Biomedical Devices and Imaging Technology. He is a fellow of the American association for the advancement of science (AAAS) and five other professional societies. He has received 5 new technology recognition awards from NASA in the fields of advanced laser imaging and nanotechnology. He was a recipient of R&D 100 award for the design of a 3D imaging system in 2008. He is involved in multiple hi-tech startup companies. His research includes diverse topics and technologies, for example, Bio-inspired Design and Engineering, Imaging, Cardiovascular Research, Wind & Sea, Energy, Carbon Nanotubes, and Micro/Nano-Scale Mechanics, among others. He received his B.S. degree in Mechanical Engineering from Tehran University (1975) and then pursued his graduate studies at Syracuse University (M.S., 1978, Aerospace and Mechanical Engineering) and Caltech (Ph.D., 1983, Aeronautics).

Mr. Robert L. Davis has been General Counsel and Senior Vice President of Glaukos Corporation since June 2015. Mr. Davis served as a Senior Vice President of Corporate Development of Meade Instruments Corp., from March 1, 2003 to April 25, 2008 and also served as its Senior Vice President of Sales from July 2006 to April 25, 2008. From April 2008 to May 2015, Mr. Davis served as senior vice president, general counsel and secretary for Targus Group International, Inc., a global designer, manufacturer and distributor of cases and accessories for mobile electronic devices. Mr. Davis served as a Vice President and an Assistant General Counsel of Meade Instruments Corp., since December 1999. He served as an Attorney of O'Melveny & Myers LLP from September 1996 to December 1999 specializing in Corporate and Securities Law. He worked as an Attorney of Morrison & Foerster LLP from August 1994 to September 1996 specializing in Corporate Finance and Labor and Employment Law. He received a BA degree in English and a JD degree from Brigham Young University.

Mr. David Haffner has been Senior Vice President of New Technologies at Glaukos Corporation since February 2017. Mr. Haffner has been on the senior management team of Glaukos since its inception in 2001. Mr. Haffner leads the critical role of developing the multiple configurations of the iStent micro-bypass product line. Mr. Haffner served as has been Vice President of Product Development of Glaukos Corporation from February 2001 to February 2017. He has more than 20 years of experience in medical device engineering, design, manufacturing, project and product management. Mr. Haffner has directed several multimillion dollar programs involving the development and commercialization of ophthalmic products and technologies targeted at cataract, glaucoma, dry eye and nasal disease groups. Mr. Haffner spent more than 10 years at Allergan, leading teams and departments in several divisions including Surgical Products, Pharmaceuticals and Worldwide Operations. Mr. Haffner earned a BS from California State University, Long Beach.

Dr. Hal Heitzmann, Ph.D. has been Senior Vice President of Applied Research and Engineering at Glaukos Corporation since February 2017. Dr. Heitzmann joined Glaukos in 2007 and is responsible for continuing applied research activities. Dr. Heitzmann served as Vice President of Applied Research at Glaukos Corporation from January 2007 to February 2017. He served as Vice President of Research and Development of EndiCOR Medical, Inc. Dr. Heitzmann has 25 years of experience in the development of medical devices at five startup companies, including Edwards Lifesciences. Dr. Heitzmann was Vice President of Research and Development with Cardiovascular Dynamics, Cardiac Pathways and with InnerSpace Medical. Dr. Heitzmann also held engineering management positions with Baxter Healthcare. Dr. Heitzmann holds a doctorate degree in molecular biophysics and biochemistry from Yale University.

Dr. Jeff Wells, Pharmd has been Senior Vice President of Regulatory, Quality and Clinical Affairs at Glaukos Corporation since September 2008. Dr. Wells has extensive experience in pharmaceutical and medical device clinical affairs, regulatory affairs, research and development, marketing, sales and business development, with over 15 years in ophthalmology. He served as Vice President of US Research and Development at Santen and also held various positions including: Vice President of Clinical and regulatory Affairs and Project Management for the US and Europe; Vice President of US Clinical Affairs and Director of Marketing. Prior to Santen, he held various marketing positions at Bausch & Lomb, where he launched Lotemax and several other ophthalmic products. Dr. Wells earned a PharmD from the University of Florida and a master's degree in management from the Kellogg Graduate School of Management, Northwestern University.

Mr. L. Jay Katz, MD, FACS, has been the Chief Medical Officer of Glaukos Corporation since February 22, 2017. Mr. Katz serves as Professor of Ophthalmology at Jefferson Medical College, Thomas Jefferson University, Philadelphia, Pennsylvania. Mr. Katz serves as a Director of the Glaucoma Service, Wills Eye Hospital. He serves as a Member of the Scientific Advisory Board of Danube Pharmaceuticals, Inc., Inotek Pharmaceuticals Corporation and Glaukos Corporation. Dr. Katz has published more than 120 articles in journals such as Archives of Ophthalmology, the American Journal of Ophthalmology, Ophthalmology and the Journal of Glaucoma. He is an editorial board member for the njournals Survey of Ophthalmology, the Journal of Glaucoma, Clinical Therapeutics and Graefes Archive of Clinical and Experimental Ophthalmology. He has authored, coauthored or edited more than 30 books and book chapters. He served as a Member of Scientific Advisory Board at Glaukos Corporation. Dr. Katz has been the recipient of several awards, including the American Academy of Ophthalmology (AAO) Senior Achievement Award in 2002 and the Joint Commission on Allied Health Personnel in Ophthalmology (JCAHPO) Faculty Award in 2003. He was named Distinguished Alumnus of the Yale University Eye Center in 2003 and became an Honored Life Member of the Philadelphia Ophthalmic Club in 2007. Dr. Katz received his MD degree from the Yale University School of Medicine. He completed an internship in internal medicine at the University of Virginia, a residency in ophthalmology at Yale University and a fellowship in glaucoma at Wills Eye Hospital. He holds a B.A. (summa cum laude) from Case Western Reserve University.
