Coworkers are intelligent individuals who become demoralized by poor managers.

Employees should be free to speak up about their managers without fear of retribution or losing ones employment.
Unchecked managers and favoritism. A survey should be created for teams to rate their managers at least twice a year.
Very straight forward. They want to know what you bring to the table and where you see yourself in 5 years.
Treat people fairly, hold proper exit interviews, promote from within rather than hire externally at lower salaries once all of the hard work is done. Encourage internal movement. HR should do more to improve its retention efforts and not be pressured by poor managers to exit employees.
