Increase honesty. Not lying workers (i.e. when it comes to equities).
I didn't liked anything specific. Skillset of the people in the interview was not the required.
Changing the CEO and HR/VP would be a smart first step. Next I would dismantle some small isolated "kingdoms" in the company and would create a more cooperative culture, next I woul change recruitment policy: it is preferably a good experienced engineer than 5 interims... And would be transparent.
