Ms. Teresa F. Sparks has been the Chief Financial Officer and Executive Vice President at Surgery Partners, Inc. since April 2015. Ms. Jennifer B. Baldock has been General Counsel and Secretary at Surgery Partners, Inc. since April 2014 and serves as its Senior Vice President. Ms. Baldock is a co-owner at Jennifer & Henry Baldock Jtyros Trust. Ms. Baldock served as an Assistant General Counsel for both Ambulatory Services of America and Renal Care Group. Prior to that, she practiced law with Waller Lansden Dortch and Davis in Nashville, Tennessee, concentrating in corporate law with an emphasis on healthcare mergers and acquisitions. Ms. Baldock is also a Certified Public Accountant (inactive). She holds a Bachelor of Arts in Economics and Accounting from Lipscomb University and a Juris Doctor from the University of Alabama.

Mr. Clifford G. Adlerz was Interim Chief Executive Officer of Surgery Partners, Inc. Mr. Adlerz has been an Interim Chief Executive Officer and Director of Surgery Center Holdings, Inc. since September 2017. Mr. Adlerz served as the President of Symbion Inc. from May 2002 to November 3, 2014 and served as its Chief Operating Officer since its inception in 1996 until November 3, 2014. Mr. Adlerz served as President of Symbion Healthcare. While at Symbion, Mr. Adlerz oversaw a range of responsibilities including corporate strategy, business development, capital allocation and various cost efficiency initiatives. Mr. Adlerz has over 20 years of operational and executional experience. Mr. Adlerz also served as Secretary of Symbion Inc. from 1996 to May 2002. He served as Regional Vice President of Midsouth Region for HealthTrust Inc. from 1992 until its merger with HCA Inc. in May 1995. He served as Division Vice President of HCA until September 1995. He served as Chief Executive Officer of South Bay Hospital in Sun City, Florida from 1987 to 1992. He was an Associate Administrator at Bayonet Point/Hudson Medical Center in Hudson, Florida from 1985 to 1987. Prior thereto, Mr. Adlerz was Assistant Administrator at North Beach Medical Center in Fort Lauderdale, Florida. He served as a Director of Symbion Inc. from 1996 to November 3, 2014. He served as a Director of Symbion Holdings Corporation until November 3, 2014. Mr. Adlerz served on the Board of Directors for the National Ambulatory Surgery Center Association and was part of the leadership group for ASC Quality Collaboration. Mr. Adlerz holds a B.A. in Business and an M.B.A. from the University of Florida.

Mr. Dennis Dean has been a Senior Vice President and Corporate Controller at Surgery Partners Inc since April 2015. Mr. Dean previously served as Vice President and Corporate Controller at Symbion Holdings Corporation and Symbion, Inc. from January 2008 to November 2014. Prior to joining Symbion, he co-founded Resource Partners, LLC, a healthcare-focused financial consulting firm and began his career at Deloitte & Touche LLP. He is also a Certified Public Accountant. He holds a B.S. in Accounting and an MAcc from Western Kentucky University.

Mr. Brandan Lingle serves as Group President of Ancillary Services at Surgery Partners, Inc. since 2018. He served in various capacities since joining the company in 2013, most recently as Senior Vice President of Corporate Services. Prior to joining Surgery Partners, he was a healthcare investment banker at Jefferies and completed the Graduate Talent Program at UBS.

Mr. George M. Goodwin has been American Group President at Surgery Partners LLC since September 2009. Mr. Goodwin served as the Chief Development Officer and Senior Vice President of Symbion Inc., (formerly, Symbion Healthcare, Inc) since September 2007. He served as Vice President, Mergers and Acquisitions of Symbion. Prior to joining of Symbion in 1998, Mr. Goodwin served as the President and Chief Executive Officer for American Pathology Resources, Inc. Prior to assuming this position, Mr. Goodwin managed the homecare operations for the Tennessee Division of HCA consisting of 22 hospitals in Tennessee and Kentucky. Mr. Goodwin serves as a Director at Adventist Health Gresham Surgery Center.

Mr. Anthony W. Taparo, also known as Tony, has been an Atlantic Group President of Surgery Partners LLC since its original formation in 2006. Mr. Taparo serves as a Vice President and Director at Pickaway Surgical Center, Ltd. He serves as an Executive Director of Arc Kentucky LLC. He served as Regional Vice President for Symbion from 1999 to 2006, focusing on specialty markets. Prior to joining Symbion, Mr. Taparo served as the Chief Operating Officer of Columbia Physician Services, a subsidiary of Columbia/HCA Healthcare Corporation.

Mr. Bryan S. Fisher has been the President and Chief Operating Officer of National Speciality Hospitals, Inc. (formerly National Surgical Hospitals, Inc.) since August 15, 2011. Mr. Fisher served as Executive Vice President and Chief Financial Officer of National Surgical Hospitals, Inc. until August 15, 2011. Mr. Fisher served as Chief Financial Officer of National Surgery Centers Inc. since 1996 and served as its Senior Vice President of Finance, Treasurer and Secretary since 1997. Mr. Fisher joined National Surgery Centers Inc. in 1991 as Controller and served as Vice President from 1993 to 1997. Mr. Fisher served as an Accounting Manager and Assistant Controller for Medical Care and prior to that, an auditor for KPMG Peat Marwick from 1984 to 1989. Mr. Fisher is a graduate of Brigham Young University and is a Certified Public Accountant.

Mr. Benjamin F. Jacobs, also known as Ben, has been Chief Development Officer & Group President of Surgery Partners at Surgery Partners, Inc. since August 31, 2017. Mr. Jacobs serves as Chief Development Officer of National Specialty Hospitals, Inc. Mr. Jacobs serves as Chief Development Officer for National Surgical Healthcare. He served as Senior Vice President of Acquisitions at National Specialty Hospitals, Inc. Prior to joining NSH in 2014, Mr. Jacobs served as Vice President of Development for Surgical Care Affiliates, Director of Mergers and Acquisitions for DaVita Healthcare Partners and Vice President of Development for CVS Caremark Corporation. He began his career as a corporate attorney at the law firm of Jones Walker in New Orleans, LA. Mr. Jacobs holds a B.A. from Washington & Lee University and a J.D. from Tulane University Law School.
