Mr. Robert S. White, also known as Bob, MBA has been Senior Advisor of Pappas Ventures since joining in 2014. Mr. White has been the President of Entellus Medical, Inc. since November 2014 and its Chief Executive Officer since April 03, 2015. He served as the Chief Operating Officer at Entellus Medical, Inc. from November 2014 to April 03, 2015. He served as the Chief Executive Officer and President of TYRX, Inc. from January 2010 to 2014. He held several positions with Medtronic from February 2003 to November 2009. He served as the President of Medtronic Kyphon from April 2008 to August 2009. During his time at Medtronic, Mr. White served as the President of Physio Control, Chief Development Officer responsible for corporate development activities and Vice President of U.S. Sales and Global Marketing, where he was responsible for commercial operations for the Medtronic Cardiac Rhythm Management business. Mr. White was employed at HyperBranch Medical Technology, Inc. He began his career at General Electric. In 1989, he joined Eli Lilly and through successive promotional opportunities served as an International Marketing Manager at IVAC for three years. Mr. White served as a Vice President of Corporate Development at ALARIS Medical Systems. At Instromedix-LifeWatch, he served as Chief Executive Officer and President. He was the Founder and Chief Executive Officer of Liquid Borders. He has been an Independent Director of AtriCure, Inc., since March 2013; Novadaq Technologies Inc. since September, 2014 and Entellus Medical, Inc. since April 3, 2015. He has over 20 years of senior management experience in the medical technology industry through numerous positions of increasing responsibility with Medtronic Inc., Instromedix-LifeWatch, ALARIS Medical Systems, Eli Lilly and General Electric. He serves as a Director at TYRX, Inc. He serves as a Member of Management Advisory Board at Topera, Inc. He served as a Member of Advisory Board at Topera, Inc. Mr. White holds an MBA from the Cornell University, and a BS Degree in Aerospace Engineering from University of Missouri, Rolla.

Mr. Michael Harry Rosenthal, also known as Mike, has been Chief Operating Officer of Entellus Medical, Inc. since November 1, 2017. Mr. Rosenthal served as General Manager of Entellus Medical, Inc. from July 13, 2017 to November 1, 2017. Mr. Rosenthal served as Chief Strategic and Technology Officer of Spirox, Inc from May 2015 to July 2017. Mr. Rosenthal served as General Partner of D3DC, LLC from December 2012 to May 2015. Mr. Harry served as Vice President of TauTona Group. Prior to joining TauTona, Mr. Rosenthal held positions at Sawtooth Labs and FoxHollow Technologies. He has over fifteen years of experience in product design, project management and business development at early stage to mid-size medical device companies. His technical expertise includes extensive experience with catheterization, micro-surgical tooling, diagnostic imaging, mechanical design, fiber optics, software development and electro-mechanical systems. Mr. Rosenthal is an accomplished inventor with over 23 issued and pending patents. He holds a B.S. in Mechanical Engineering from Stanford University.

Mr. Brian E. Farley served as Chief Executive Officer of Entellus Medical, Inc. from March 2010 to April 01, 2015 also President from March 2010 to November 2014. Mr. Farley served as a Strategic Financial and Business Consultant at Cardica Inc. since January 2010. He has 30 years of medical device experience and management expertise. He served as the Chief Executive Officer and President of Vnus Medical Technologies Inc., from January 1996 to June 2009. He joined VNUS Medical Technologies, Inc., as General Manager and the First Employee in July 1995. Prior to VNUS, Mr. Farley was employed from 1981 to 1995 at Guidant Corporation, in the Medical Device Division of Eli Lilly and Company and in Lilly Research Labs in a variety of research and development, clinical research and business development leadership positions. Prior to 1996, he was employed in various management and executive positions in research and development, clinical development and business development by Eli Lilly and Company its medical device division and its spin-off company, Guidant Corporation. He held various management and executive positions at DVI. He served as the Chairman of Neuronetics Inc., from June 2011 to July 2014 and again since December 2014. He served as the Chairman of the Board at Entellus Medical, Inc., since November 2014. He has been a Director of Entellus Medical Inc. since November 2008; Neuronetics, Inc., since November 2009 and DFine, Inc. since March 9, 2012. He served as an Executive Director of VNUS Medical Technologies, Inc., since January 1996. He serves as a member of the advisory board at the Purdue University Biomedical Engineering Department. He is a Member of the American College of Phlebology Foundation Board of Directors. Mr. Farley holds a B.S. degree in Engineering with a major in Biomedical Engineering from Purdue University. He has an M.S. degree in Electrical Engineering with a major in Biomedical Engineering from the Purdue University.

Dr. Donald A Gonzales, MD, has been the Chief Medical Officer of Entellus Medical, Inc. since July 13, 2017. Dr. Gonzales belongs to a rare class of clinical entrepreneurs who successfully blend their clinical experience and business outlook to address unmet needs within the medical community. He founded ENTrigue Surgical, Inc. in 2007 and also served as its Chief Medical Officer from March 2007 to April 2011. In 2004, he formed the first of several companies including Xorbent Technologies, Xorbent L.L.C. and Xorbent, Inc. He was a General Partner for D3DC, LLC from December 2012 to May 2015. He served as a Consultant for Entrigue Surgical, Inc. from May 2011 to April 2012. From May 2015 to July 2017, he served as Chief Medical Officer of Spirox, Inc., a medical device company acquired by Entellus in July 2017. He has also performed cutting-edge research at the Cardiovascular Research Center at Massachusetts General Hospital, Memorial Sloan Kettering, Strang Cancer Institute at Rockefeller University and Cornell University. Dr. Gonzales has also had active clinical practices as a Staff Physician at Tulane University and in private practice in Beverly, MA. He is also listed as an Inventor on six patent applications for devices and methods in ENT surgical applications. He serves as a Director of ENTvantage Diagnostics, Inc. and ENTrigue Surgical, Inc. Dr. Gonzales graduated from Gordon College and Cornell University Medical College. He completed his residency in Otolaryngology-Head and Neck Surgery at Tulane University School of Medicine. He holds a Doctorate in Medicine from Cornell University.

Brent Moen serves as the Chief Financial Officer of Entellus Medical. Brent started at Entellus Medical in May of 2016. Brent is currently based in the Greater Minneapolis-St. Paul Area.

Nicholas Binge serves as the Area Vice President - Sales US West of Entellus Medical. Nicholas started at Entellus Medical in January of 2018. Nicholas is currently based in the Greater Denver Area.

Karen Peterson serves as the VP Clinical, Regulatory and Quality of Entellus Medical. Karen started at Entellus Medical in May of 2010. Karen is currently based in the Greater Minneapolis-St. Paul Area.
