In depended on the state. California's government required some weird crap with paying PTO or some stupid crap. I was in Oregon. There you got 3 weeks per year, completely flexible, approved by your manager only, and it was only tracked informally, usually on personal spreadsheets. You get 4 weeks once you've been there 10 years. It is use or lose it, which I liked because their attitude was USE IT!
You get additional days according to seniority; up to 20 days per year (including days required by law); there is a use it or lose ir policy.
It's pretty typical. 3 weeks for the first 10 years then 4 weeks. All flexible between you and the manager, no central tracking.
Decent, it is flexible (officially uses the term 'about' X weeks, but base number of days increases slowly.
3 weeks per year for salaried engineers, increases to 4 weeks after 10 years at company.
Excellent. Sabbaticals every 4 years and 2-3 weeks vacation every year.
