What does the leadership team need to get better at?

Communication. There is no communication even among management members, much less the workers who the decisions affect.

Communication and training. Employees need to be properly trained instead of thrown in to sink or swim.

What needs to change to make the company culture better?

Management and decision making needs to actually care that their employees have lives and families and respect their time.
