Very small team that puts out high volume of work with very little employees

Just some of the co-workers that I work with good people who work hard and deserve advancement
What's going wrong and how can it be improved?
I really don't know how things can be improved when you bring up situations they're swept underneath the rug and when you're honest it kind of digs a hole to the employee
What does the leadership team need to get better at?
Understanding employees and they say that there is always an open door policy but then when you tell the truth about things it does digs a hole for yourself and turns out to be bad for the employee
