Accountability and integrity. They need to lead by example.

Friendly interviewers. They do need to be cautious about bias in interviewing.

Better benefits with 100% of premium covered. Not doing 3 people's jobs for one person's salary.

What's going wrong and how can it be improved?

Lack of manager accountability or training. When HR implements training programs, problems are not addressed individually with the managers, leading to a culture of "this isn't relevant to me and I'm only here because I have to be"
