Any level of accountability for management -- at present there is none at all, they have a free pass to do nothing, including simply not coming to work or making minimal effort at their jobs

What's going wrong and how can it be improved?
When narcissists and sociopaths are in charge, the effects are vicious, widespread and all-consuming -- HR is oblivious and unresponsive to this. No wonder the company is failing.
