Certain areas of the code (FE) lack expertise and motivated key players. Expected velocity is growing faster than natural expertise/familiarity growth on some teams. The difference in output between key players and other team members is often dramatic (10 to 1).

There is a severe skill/knowledge disparity that leads to no significant team participation in planning. Also, the project planning that is upstream of development is slower than the development and this leads to grasping for the next work items OR working on incomplete info.

It's good that the requirements definition and planning feels like it is done entirely at the team level. However, that also makes it seem like every PO is an island and if they're not doing a good job integrating the whole program may be chaos when seen together.
