300 words isn't enough and I haven't formulated a concise fix yet. It's to do with the rigidity of work units and the ability to inspire quality, consistent and repeatable work. Humans are variable and the risk management of the model is not taking account of that.

The overwhelming, and obvious, feature that results is that good people are overloaded and they, along with many others do just enough to move things off their desk rather than finish it
