Be people driven and implement culture and processes. Always trying to put out fires which puts everyone behind. Also letting go of good people and keeping the ones that clearly have no vision, don't communicate well with others, and micro manage.

It is pretty apparent that the C-Level executives favorite employees are the ones who stay the latest. The quality of your work is important, but unfortunately how late you stay appears to be more important to them.
