Accountability and transparency. We just keep hearing "we need to make hard choices." You burn people out with having approach resources and wonder why key people leave. You have cut too deep and by the way how about having an approved budget before a 1/4 of the way through the year.
No clear vision or strategy. Company has been influx for over a yr. Leadership is in remote locations and rarely present.
