People are unhappy and burnt out. Leadership is either blind to, or doesn't care about, well-being.

Listening to and recognizing the practitioners. They pat themselves on the backs for the hard work (and late nights) of lower-level others.

What needs to change to make the company culture better?

Investment in the people. Realistic expectations for work turnaround so people are not functioning at backbreaking speed and capacity. Keep the people happier and you won't keep losing them.
