Raises are not given appropriately, or timely, or based on performance. Merit increases across the board that are below the rate of inflation don't count. Toxic management that plays favorites and gives undeserved promotions are an obvious issue.

Consistently exceeding expectations and not getting acknowledged for it, no raises (except across the company merit), no promotions, etc. An AD that consistently tries to undermine the work of the team because he is only interested in making himself look good to the ELT when he is underqualified.

I think that people sometimes do not know what an appropriate work conversation is. I have heard distasteful comments, political squabbles, and significant negative talk about other people and or departments. Intelligence is commonly insulted, and I do not find that productive.

Not sure but there is a growing belief that upper management is not doing a good job. From my perspective it seems as though decisions are being made rashly and without foresight. I think there's a feeling that people aren't being heard.
