Managers should be anonymously evaluated twice a year by every employee they manage. There should be anonymous surveys done by a third party company so that employees can express their concerns and situations can be resolved before it's too late.
Communicate and stop avoiding questions. Be clear about your intentions. Tell us why youre doing things this way and how it should benefit the company forward. Open paths for people to work in teams based in other regions.
The CEO is extremely temperamental and the leadership team lives in fear of him. Certain employees have targets on their backs seemingly because the CEO simply doesn't like them. Other employees get nonstop opportunities & promos because he "sees himself in them" (almost always young men).

Eliminate middle managers, hire leaders who know what it is that their team does and have a career path for the ambitious.
Have leaders that know what we do on a day to day basis. Know what our customers need and want. Move team members into manager roles, because they are the only ones who know what our products really do.
Please keep an eye out on middle tier managers (and their managers who support them) who bully your employees.
HR isn't there to help employees. They are there to protect the company.

It really weeds out people that would not be a good fit. There is typically a case assignment that will ferret out people's business / technical / and customer facing skills. That helps ensure you are working with great people.

Speed up the process and don't draw it out over months. Have people who actually know what we need interview the people. Quit hiring people from a certain company that already have a relationship with the leadership. No one wants a duplicate of that company....

Projects are bid so low you are over budget and out of time before you even start. The leadership team has no clue what all goes into a project. There is no work life balance for non-managers and no career path. Teams are expected to give everything to a project with no long term goals.
