There is a strong blame culture at Axtria. When things go wrong, the first reaction is to find someone to blame the issue on instead of identifying and fixing root cause

The offshore teams do not have enough ownership or accountability for their work. This leads to major lapses in quality - the issue is company-wide. Management is too focused on selling projects instead of providing guidance
