Define a process and follow exactly. Issues are not w.r.to the skill but a huge lack in defining role and responsibilities . Follow the process

The company CEO is very good. Down to earth person. He is very motivating.
The Organisation is well connected with each other co-owners. It is easy to contact the required person when you are in trouble. A Fresher and a Vice president is treated with Same respect.
Manager should identity the employees talent and should give promotion based on that. Not whom he or she likes.

Leadership team should focus on getting new project so that when one project shuts down, those employees do not lose job rather get job in other projects. You have to ensure the job security

Too many management layers, Disoriented, unqualified directors and managers leading accounts with no leadership skills. Need to infuse energy into system to gain trust and confidence of employees. Provide platform for growth.
