What needs to change to make the company culture better?
change all human resources staff, manager and team leaders
Do employee performance reviews. So the employee is informed about procedural changes and expectations and understand/ discuss individually there performance of required job duties.

What's going wrong and how can it be improved?
First the way in which the company presents itself to the customer and potential employees is in steep contrast to the reality. I find that there are avoidable instances if we had people that were better skilled in time management and people skills
