The leadership team needs to ensure that their new hires are embodying company values. New hires should be hired at the appropriate level compared to existing employees. Newly hired leaders should not be allowed to just bring over all the people from their previous company.

There's no negotiation for salary, so everyone ends up getting paid the same, independent of their contribution or ability to contribute.
There needs to be a effort on the part of management to hire people who can collaborate effectively. We have some smart people on our teams, but they also tend to be arrogant and unpleasant to work with.

We tend to eliminate anyone who doesn't fit the mold of a few people on the team. There's a presumption that new hires should just be code monkeys with nothing new to contribute.
