Leave the silos and egos behind the door. Stop thinking of yourself as a part of team A or team B. You should be the part of the company and care what other departments/teams are working on. Separate and not synced OKRs are disaster.

No line of communication between PMs, leadership, engineering and design. PMs do not do their job and do not have an understanding how to make the product better for the customers because they do not listen to designers and how to design it with scale in mind because they do not listen to engineers
