What's going wrong and how can it be improved?

There seems to be a lot of anxiety and tension and fear of "messing up" and that creates paralysis that I think adversely impacts performance.

ACH and Credit cards are fundamentally flawed because the secret number that authorizes control of funds is impossible to keep secret (you share it with people in order to spend money). Our industry is built around pretending we can keep such payment channels safe. Cryptocurrencies will crush us

when I started at this company 20+ years ago, I felt like I had found the perfect dream employer, but not any longer, with each management change I see more and more changing and not for the better, except for the folks at the director level and above....of course !
