Recognizing the massive sea change regarding remote work. Very much stuck in the past when it comes to this.
Make sure people's roles are clearly defined. Multiple groups trying to accomplish the same thing in competition with each other causes massive problems. Huge waste of resources and creates toxic competition.
Need to improve the culture around meetings and collaboration in general. Meetings rarely have even the most basic agenda and are often a free-for-all for random ideas, and suggestions. 10+ meetings in a day is pretty common. Not much ever comes from most meetings, either. Massive drain.
Pay more attention to department structure and focus on the output of the work, rather than coming up with ideas. We're drowning in ideas, but lack competent "doers" to get the work done. Off-shoring work needs to improve, too. Simply hiring someone in India doesn't get the job done.
