the welcoming attitude for updates and feedback alike

What's going wrong and how can it be improved?

our technical debt is staggering, we do a poor job of service assurance, quality control, and monitoring. monitoring, reporting, and telemetry are insufficient, and no standards and practices are defined. we have intelligent highly trained people with few, poorly maintained tools.
