What needs to change to make the company culture better?

Involve employees more in large decisions. We say we do, but we don't. Example: all decision making around covid was done in a total vacuum.

What's going wrong and how can it be improved?

Not a place for employees to get better. The focus is on making money, not on improving people - If you're lucky that might be a side effect. This is mostly the 'survive or be killed' mentatlity of DoD contracting. We have no room for creativity - if it's not a requirement, we can't do it.
