They hire the most incompetent people and put them in leadership positions. They don't promote from within unless it's someone who can't do their job and isn't a leader. Managers who can't do their job will put a target on your back to make themselves look better. No communication. Blaming game.
What needs to change to make the company culture better?
Open communication. Firing management who are antisocial and lock doors on team members. Firing people performing sexual acts on the clock
