Management. Especially one of our managers in specific. I think reviewing who's deserving of management positions should be the start.
Leadership is not willing to change. They look for the negative. And always find it. They don't listen to the team. They make excuses for when they do things wrong or they yell at people. They don't approach the situation with a question. But with a you did this wrong. They need to learn to listen.

Lack of culture, lack of leaders. Get a new administration and people trained to be in higher positions.
Leadership doesn't take their jobs or positions seriously, but reprimand the lower rated employees if they don't give 110% of themselves. It's an exhausting cycle of fake leadership and overworked team members
Management. The right example could be set. Proffesionalism. Better at hiring the right workers. Hostile environments not being created by leadership and being enabled by leadership to regular workers.
