Better organizational processes, we depend to much on POC and not in standardized & clear processes (ER processes, HR processes, TA selection processes & compensation) the organizational skills from many team leaders are not developed.
the leaders of each team are being selected as per the perception that the selection panel has, and not structured tests that allow them to actually understand the skills of the person. It can be very bias.
Organizational and time management skills are very weak within the company. It is perceived that the more time that you spend in meetings the more productive you are, which is incorrect. And operational processes are not documented from team leaders at all.
