Most people seem to want to succeed, but can lack the tools or motivation to make it happen.
What does the leadership team need to get better at?
Be more clear with communicating how we will close the gap between lofty goals and where our current capabilities are.
What's going wrong and how can it be improved?
There seems to be a disconnect between what the leadership thinks is happening and what is actually going on. There seems to be a culture of telling leadership what they want to hear. It could be improved if more of the leadership team led by example and actually dug in and understood the issues
