What does the leadership team need to get better at?

They need better management strategy instead of pressuring the agents.

What's going wrong and how can it be improved?

Getting things changed so fast, process can be too smooth but the management thinks that the process is too easy and every one can easily work this approach is not helping the team even this is the main reason that employees leaving the job. So just manage the thinks not pressured people to work.
