What's going wrong and how can it be improved?

Zero Accountability, No communication from Director+ level down to middle management. Director level does not properly communicate plans, future or current, to manager and below. "Collaborate then commit" is absolute lip service.

What needs to change to make the company culture better?

Clear gap between the Director level and Middle management. Often times, Managers are not kept in the loop on future plans and direction of ongoing projects, especially in supply chain. Zero Accountability at any level. Directors need to communicate better, plain simple.
