we lack direction and focus, we all work hard and often long. we run in too many different directions. we try to make everyone happy instead of prioritizing and making trade-offs, firing customers that aren't worth the trouble, strategically ceasing investment in losing areas.

The work site is poorly maintained, potholes, leaking buildings filled with vermin, trees down in the roadways that aren't cleared for days or weeks. Stop wasting money on reconfiguring the conference rooms we can't even use and fix the infrastructure first!

Lack of leadership skills in people who are formally assigned team lead/manager roles. More training and translation of mission/vision/values into concrete and visible ways of working (processes, standards, etc.) would help improve this. Need leadership to exemplify best practices & set the example.
