In my experience there are several managers who have reached their level of incompetence. They don't know what is going on and they are afraid to make any changes. There is no incentive (salary increases) to improve so they seek the status quo.

Have an agenda. Have accountability. Avoid having irrelevant parties in meetings.

Lack of accountability. Lack of responsibility. Allow people to innovate and be responsible. Stop finger pointing.

What needs to change to make the company culture better?

Reward innovators and people who take responsibility. Allow for mistakes. There is a culture that doesn't want to do anything for fear of doing something wrong.

What does the leadership team need to get better at?

Accountability. There is a culture of finger-pointing. There is a freeze on raises and people are resentful and don't try to innovate. People frequently (and blatantly) take responsibility for work which is not their own.
