What does the leadership team need to get better at?
Get better at incentivizing people who generally put effort into the company and have a lengthy stay. They are there for a reason, don't shut them out as if they are a newly bad employee. Have better peer reviews on terminations.

Benefits are too expensive. Bonuses are cut because of bad management decisions.
