Executive Management, Senior Management, Operational Management, Area Management ... Oh, and nepotism & cronyism need to stop!

Management is the problem, they're bringing in people off the street and placing them in roles they do not have the skills or experience to do.
What does the leadership team need to get better at?
Realizing they employ human beings, not machines. If you cannot treat employees you actually see every day with respect how do you obsess over customers you never see?
